The organizations below are the providers that you will find right inside our doors at CONNECT Allen County. This list is just the start of our partnerships throughout the community. Onsite Partners:

Choices Coordinated Care Solutions, Inc.

Choices Coordinated Care Solutions is a national nonprofit organization committed to supporting youth, adults, and families facing significant behavioral and emotional challenges. In Fort Wayne, Choices focuses solely on providing intervention based services that are court ordered. With this in mind, Choices was selected to be the neutral party between our funding source and our partnering agencies acting as the hiring agent for our CONNECT staff members. This means that any referrals that come through CONNECT Allen County are navigated from the CONNECT staff directly to other on or off site partners and Choices does not service any of these referrals. Crossroad Child & Family Services, Inc.

Crossroad creates promising futures for children in difficult circumstances by providing therapy, education, and family support services. Crossroad services include case management, wrap around services, residential treatment and counseling. While Crossroad has a strong focus on serving children and families, they also have programming that includes serving adults in need of mental health counseling as well. Parkview Behavioral Health Institute

One in five American adults will experience a mental health condition in any given year. Parkview Behavioral Health, believes that everyone deserves compassionate care, unwavering hope and unconditional encouragement. Their dedicated team of behavioral health specialists provides expert mental well-being and addiction services to individuals and families in the communities they serve. The Otis R. Bowen Center

Bowen Center offers a full range of professional services to meet the total health needs of all Hoosiers. Their dedication to providing extraordinary, confidential, life-changing care to individuals and families means they walk shoulder to shoulder with their patients to help them live their best life. Their team of highly trained professionals treat the whole person providing mental and physical health care, therapy, life skills coaching, treatment for Substance Use Disorder and more. SCAN, Inc.

SCAN’s mission is to protect children, prepare parents, strengthen families and educate the community to Stop Child Abuse and Neglect. SCAN offers a wide array of programming including court ordered intervention services and referral based prevention services. You can learn more about their services at www.scanfw.org.


Bienestar Sin Fronteras

Bienestar sin Fronteras provides FREE mental health services to the uninsured Latino population in northeast Indiana in their native language. If Spanish speakers are insured and searching for therapy services in their native language Bienestar will also work with the family to help them find a therapist that can serve them on their current insurance plan. The Rescue Mission

The Rescue Mission exists to provide, through the power of Jesus Christ, a home for the homeless, food for the hungry, and hope for the future. Some of the services offered by the Rescue Mission include free meals daily, emergency shelter for men experiencing a housing crisis including a clinic on site, emergency short and long term housing for men experiencing homelessness, and short and long term housing for women and children experiencing homelessness. Catholic Charities

Catholic Charities Diocese of Fort Wayne-South Bend is a social services agency assisting those in need as Christ would do. Catholic Charities offers a wide range of programming including rent, utility, and food assistance, case management for pregnant and parenting teens, medical interpretation for Spanish speaking individuals, senior employment programming, refugee support services, and immigration services. Lutheran Social Services of Indiana

LSSI focuses on promoting the overall health of the community, equipping willing families facing the most challenging obstacles with skills necessary to make positive life choices as evidenced by advancement toward economic and family stability. LSSI provides a wide range of holistic services, including home-based case management, mental health and recovery care, financial stability support, employment services, teen education and advocacy, child care and more. Building a Stronger Family

Building a Stronger Family is an organization that has been formed for one mission, to build a stronger community by building stronger families. BASF feels this mission can only be accomplished by focusing on multiple areas that impact the well being of the family structure. BASF strongly believes that a community is only as vibrant and strong as the families that make it up. Meridian Health Services

Meridian Health Services is a progressive healthcare organization that believes in treating the “whole-person” integrating physical, mental and social well-being to help people achieve their optimum health. This approach connects treatments and doctors working together to heal both the body and mind for total well-being. Journey Birth & Wellness

Journey Birth and Wellness comes alongside women from all cultures and communities during pregnancy, labor and early postpartum. Journey offers free childbirth education and doula services on a sliding scale fee. Journey’s goal is to reduce maternal and infant mortality in the Fort Wayne area. Everything that Journey does is to help with “Birthing a Better Fort Wayne.” (A Doula is a medical professional who provides physical, emotional, educational, and advocacy support to women during pregnancy into motherhood.) Made Strong

Made Strong serves people of all ages, genders, and orientations who work, or have worked, in any area the adult entertainment industry. We offer case management services for the health, safety, and well-being of the sex work community. New Wave Wellness

New Wave Wellness is a behavior management waiver provider through the Indiana Developmental Disabilities Waiver program. We also are here to help individuals navigate the BDDS waiver program. New Wave is also going to be providing Medicaid only counseling services here in the near future. www.newwavewellness.org
